Medical Assistance and MinnesotaCare

When you apply for financial help through MNsure, you will find out if you qualify for low-cost or no-cost health insurance, either through Medical Assistance or MinnesotaCare. These two government-sponsored programs are managed by the Minnesota Department of Human Services.

Gather your information

Download and read the MNSure Application for Health Coverage and Help Paying Costs document. You do not have to fill out this form (unless you prefer hardcopy).

The document contains the entire application, along with step by step instructions.

If you print this out, you can use it as a guide for gathering all of the information that you are going to need before you begin to fill out the online application.

Apply Online

The online application is fast and easy! You may be able to get real-time decisions using the online application.

You can also get help online if you have questions during the application process.

Gather your information

Information that will help us determine if you qualify:

  • Social security numbers (or document numbers for any legal immigrants that need insurance)
  • Employer and income information for everyone in your family. (for example, from pay stubs, W2 forms, or wage and tax statements)
  • Policy numbers for any current health insurance
  • Information about any job-related health insurance available to your family

We will keep all the information you provide private and secure, as required by law.
