Report Foodborne Illness

If you think you got sick from eating at a restaurant or other public food service:

Gather your information 

You will be asked about:

  • Your symptoms
  • Where you ate and what you ate on the day before you became sick
  • Where you ate and what you ate for three days before that

This report is confidential and can help determine if other illnesses are linked to a food you ate.

Email us 

Include the information about your illness as well as an additional way we can contact you. We will contact you to get all of the information listed in Step 1 above.
